Player Activity Statement

Members can only request their own PAS for privacy reasons. Photo ID will be required to collect your PAS from the Club once you have been contacted. 

We will hold the statement for maximum 7 day. If uncollected, after 7 days, it will be destroyed and your will need to resubmit your request.
